To prepare for your upcoming appointment with the dietitian or nurse educator, we ask that you fill out a minimum of 3 days of your usual food intake and physical activity routine. (you will have a 2 wk free trial of the Nutribasic service to complete your food and exercise information. During this time you are welcome to use the other features of the service including pre-established mealplans, medication tracker, and logs for your blood pressure and blood sugar readings. This information is electronically secure and will not be shared by anyone besides your health care provider. At your appointment, you will receive a copy of the results of your data which will be used to create personalized lifestyle modifications JUST FOR YOU!

Who Is Asking Me to Do This?
Registered Dietitian: The Registered Dietitian is uniquely trained to offer individual medical nutrition counselling to help manage diabetes, hypertension, high cholesterol and many other health issues. To help understand your usual eating habits, the food records will provide information about what types of foods you are eating, how much and when. The dietitian will be able to offer feedback as to whether you are getting enough nutrients for your medical, heath and wellness needs. The Nutribasic service may be a useful tool to use afterwards to help incorporate the nutrition guidelines that are recommended for you.
Nurse Educator: Part of the puzzle of managing some chronic diseases is the diet and exercise component as it related to blood pressure, blood sugar readings or your cholesterol levels. The Nurse Educator will focus on providing health education on a broad range of topics including those related to health promotion and disease prevention to both individuals and groups.
